Custom Survey Headers

Survey headers are customized with logos, titles, instructions, etc. using the voting rules section of the voting profile designated as the primary voting profile when distributed ballots for a survey.

User ID, Organization Level 1, 2, and 3 and up to 255 additional demographic fields may now be used in the header and instructions for a survey, as set in the primary Voting Profile. The variable fields are:

These variables may be used in both the Voting Rules and Text above voting table sections of the primary voting profile.

The display of user ID, organization and demographic data is particularly useful for surveys, such as 360 degree assessments, where each participant is asked to complete several surveys and needs help keeping track of which one they are working on.
